When you stand at the gate of a realm, enter your inner world: the old master waits for you and offers the trial. Accept, and your Heart Demon rises from black smoke: a dark copy of you, with your skin, your art, your weapon, and exactly your health, armour and damage.

How hard it hits

Its blows start at 100 % and are lighter with preparation: −30 % with a Realm-Breaking Pill, −20 % if your body sits near the sanctuary, −10 % if high up or in a Murim biome, −10 % per earlier failure. Pill toxicity above 50 adds 20 %.

Win

You enter the next realm at Early with a full reserve, and the whole server hears of it.

Lose: Qi Deviation

Defeated, leaving, or your body struck: −15 % Qi Max (you never lose a stage), an empty reserve, and for 5 minutes −15 % speed, −20 % damage, no regeneration.

Declining or leaving before the demon rises costs nothing.
